Counter-signing that’s governance, not a rubber stamp
Counter-signing is the clinical-governance core. Significant notes — concerns, NEWS2 captures, incidents — queue oldest-first for a senior carer or nurse, who must tick an attestation that they’ve reviewed the record before they can sign. No blanket bulk-approval; one record at a time.
