At shift end the system drafts a per-resident summary from the day's notes, concerns and observations. The outgoing carer reviews and signs; the incoming carer acknowledges. That signed chain is exactly the continuity-of-care evidence CQC looks for.
Two moments, one screen
Handover is two jobs in a single place — reading what the last shift left, and handing over what yours did — on a simple Incoming / Outgoing toggle.
Incoming — start of shift

Drafted from the day, not from memory
Each outgoing summary is assembled from three streams: notes the carer ticked “add to handover”, every open concern from the shift, and the latest observations such as a NEWS2 capture or a soft-sign cluster. The carer edits the draft rather than starting from a blank box.
- Every caseload resident appears — “all settled” is an explicit handover statement, not a silent gap.
- A reviewed pip per card; sign-off warns (but never blocks) if any card is unread.
- Edits autosave as you type; “reset to auto-compose” re-pulls the latest if a note arrives mid-shift.
Signed by the outgoing carer, countersigned by the incoming
A handover only counts as evidence if you can prove the information actually crossed the shift boundary.
