RESTORE2 soft signs, captured as you go
The earliest signals of decline are the soft ones — new confusion, reduced food or fluid, cold or clammy skin, new incontinence, increased pain. FlexiEle gives carers a soft-sign category so those observations are recorded in a structured, consistent way the moment they’re noticed.

NEWS2, scored by the system — not the spreadsheet
A senior carer or nurse enters the vital signs — respiratory rate, SpO₂, oxygen, temperature, blood pressure, heart rate and consciousness (ACVPU) — and the server computes the official NHS NEWS2 score with a per-parameter breakdown. The capture is stored with a paired note, cross-linked, in a single transaction.

- Role-gated — clinical observations are taken by senior carers and nurses.
- Inputs are sanity-bounded; the score and escalation tier are shown for the clinician to act on.
A clinical watch the manager can’t miss
The manager’s workspace runs a dedicated clinical-watch queue that merges two signals, so deterioration is surfaced whether or not anyone took a score.

A NEWS2 score of 5 or more
Any recent capture at medium-or-high acuity floats to the top of the queue, worst score first.

A soft-sign cluster with no score
A resident showing two or more soft signs in 48 hours but no NEWS2 — exactly the case that gets missed on paper.
Designed for clinical safety
Clinical scoring carries real responsibility, so we treat it that way. The NEWS2 algorithm is implemented once, server-side, reviewed against the published NHS chart and version-tagged under DCB0129 — the client never computes the stored score. In V1 there is no automatic escalation: the score is computed and shown with guidance, and a clinician decides what to do.
