How to use this page

24 KLOEs, five Key Questions, one reference.

CQC's draft framework groups 24 Key Lines of Enquiry under the five Key Questions — Safe (7), Effective (4), Caring (3), Responsive (4) and Well-led (6). Each entry below gives you the inspector's question word-for-word, the scope CQC will assess under it, and the part of FlexiEle that produces the evidence.

This is the draft framework, published for consultation — it may change before it goes live. We reproduce it because reading it in full helps every provider prepare, and we update this page whenever CQC revises the framework.

Rating characteristics

What Good and Outstanding actually look like.

Alongside the 24 KLOEs, the framework publishes rating characteristics — CQC's plain descriptions of Outstanding, Good, Requires Improvement and Inadequate. They replace the old numerical scoring: an inspector now weighs your evidence against these descriptions and judges it in the round.

A note on completeness. Every KLOE has its own four rating characteristics — together, several hundred descriptor passages across the draft. We publish them per KLOE once the framework is finalised, after the 12 June 2026 consultation; transcribing draft wording that is about to change would only mislead. Below is one worked KLOE — Safe, KLOE 1 — so you can see the shape.

Outstanding

"A positive safety culture is firmly embedded in the service. Safety concerns are recognised as essential to continuous learning and improvement. The service continually and actively seeks opportunities to learn from risk and safety incidents, good practice, and national safety alerts. Learning is regularly revisited and shared with partners to strengthen safety across the wider system…"

Good

"Safety is a shared priority for everyone, including staff and people using the service. There is a strong learning culture in which incidents that have caused harm, or could cause harm, are treated as opportunities to improve. Safety risks are consistently monitored, proactively identified and managed…"

Requires improvement

"Good safety practice is not consistently maintained. Staff do not consistently understand their role in proactively identifying and contributing to managing risk within the service or how they can do this. Safety concerns are not consistently identified or addressed quickly enough to keep people safe…"

Inadequate

"Safety is not a sufficient priority for the service, and the service has a track record of failing to provide good standards of safety. The service does not recognise concerns, incidents or near misses that place people or staff at risk of harm, or that have caused harm. Investigations into concerns or complaints are insufficient, delayed or not completed…"
